This web site is a new venture for the practice so we hope have found it a valuable source of medical information about us and the services we provide.

If you have any suggestions about  how improvements can be made then fill in the comments section at the bottom of this page. Alternatively you  can pass comments on to Dr Masharani via his email address below.  (Note: emails are not picked up on a daily basis so do not use this method if your enquiry is urgent.)

You can also fill in the cards provided at reception and place the completed card in the suggestion box which is situated in the waiting room at the surgery.

Complaints

We also run a formal complaints procedure and information about this can be obtained from the reception staff or you can print off the complaint form by clicking here. It is best that you post the complaint to Dr Masharani for his personal attention rather than email a complaint to him. Alternatively you can complain to your PALs representative

Change of Address

If you have moved then let us know by filling in the Change of Address form. Just check that you are still living within the practice area.

If you have moved out of the practice area, you will need to register with new GP. Asking your neighbours is a good way of getting a personal recommendation but if you want a more objective way of deciding ask the local health authority (look in the local phone book) or nhs direct web site for a list of GPs in the area and go around to the surgery and talk to the receptionists and pick up a practice leaflet. Some practices also have web sites.

When you have made your choice you will need to fill in a registration form or alternatively fill in the inside of your current medical card and hand it in to the new surgery. These days everything tends to occur electronically so you will, in effect, be instantly registered and we will receive notification at this surgery to send on your medical records to the health authority who will then process them before passing them on to your new doctor. This last process is not electronic and occurs at a snail like pace - taking about 6 weeks. If you are on regular medication you should have the right hand side of your prescription to show your new GP but if there are any problems get your new GP to telephone us.

Lastly, if you do move away from us, all the practice staff and doctors wish you well for you and your family in the future.
